L’orthobiologie et la médecine régénérative en musculosquelettique, orthopédie et médecine physique et sportive englobent l’utilisation de produits biologiques pour optimiser la réparation tissulaire, réduire la douleur et améliorer la fonction après des lésions osseuses, cartilagineuses, tendineuses ou ligamentaires.
Les principales approches incluent les injections de plasma riche en plaquettes (PRP), les concentrés de moelle osseuse (BMAC), les cellules souches mésenchymateuses (CSM) issues de la moelle, du tissu adipeux ou d’autres sources, ainsi que l’utilisation de facteurs de croissance, d’acide hyaluronique, de matrices/scaffolds et de greffes cellulaires ou tissulaires.[1][2][3][4][5][6][7][8][9][10][11][12]
Les indications cliniques les plus étudiées sont l’arthrose, les lésions cartilagineuses, les tendinopathies, les lésions ligamentaires et certaines fractures ou pseudarthroses.
Les résultats cliniques sont variables, en partie à cause de l’hétérogénéité des préparations et des protocoles utilisés, ce qui limite la comparabilité des études et la généralisation des recommandations.[1][2][3][6][8][9]
Les bénéfices démontrés sont surtout symptomatiques (amélioration de la douleur et de la fonction), avec peu de preuves robustes d’une véritable régénération tissulaire structurelleà ce jour.[8][9]
Les sociétés savantes telles que l’Orthoregeneration Network Foundation, le GRIIP (groupe de recherche international sur les injections de plaquettes) ,l’ESKKA (European Society of Sports Traumatology, Knee Surgery and Arthroscopy) soulignent la nécessité de standardiser les préparations et d’augmenter la qualité méthodologique des études pour mieux définir les indications et l’efficacité de chaque modalité.[2][4][5][7][10] Les thérapies émergentes incluent les exosomes et la thérapie génique, mais elles restent expérimentales.[8]
Enfin, il est essentiel de suivre l’évolution de la réglementation, notamment aux États-Unis où la FDA encadre strictement l’utilisation clinique de ces produits.[8]

Le GRIIP est le Groupe de Recherche International sur les Injections de Plaquettes. Il s’agit d’un groupe international d’experts qui publie des recommandations et des consensus sur les indications, les contre-indications et la sécurité des injections de plasma riche en plaquettes (PRP) dans les pathologies musculosquelettiques, en particulier chez les patients présentant des comorbidités infectieuses, oncologiques ou hématologiques.[1] Ce groupe possède effectivement plusieurs publications à son actif, dont un consensus international récent sur les contre-indications du PRP dans ces contextes.
